WELCOME! Acentra Health provides administrative review of the FSP Application and conducts clinical review of the supporting documentation to determine whether the youth meets the eligibility requirements and medical necessity for residential services.
Submit Initial FSP Applications, Continued Enrollment (renewals) and Residential Prior Authorization review requests online through eQSuite®, the FSP Web Portal.

NEW! FSP ONLINE HELPLINE - EASY TO USE
The FSP Online Helpline module in eQSuite® now provides SASS FSP Coordinators and Residential Treatment Facilities the ability to submit questions and receive a response in one, secure location. While you are online using the Web Portal, simply click Online Helpline menu tab.

GETTING ACCESS TO FSP WEB PORTAL
CCSO agencies and Residential Treatment Facilities, please complete the eQSuite System Access Form below and fax to Acentra Health to receive access.
FSP COORDINATOR CONTACT INFORMATION:
CCSO Agencies: To update the assigned FSP Coordinator (s) for your facility, please print and complete the FSP Provider Contact Information form below and fax to Acentra Health:
Upon completion of clinical review, if an Acentra Health Physician Reviewer renders a denial outcome, the Provider may request a reconsideration of that denial for the following review types:
- FSP Continued Enrollment/Renewal Application - SASS FSP Coordinator
- Prior Authorization of Residential Services - SASS FSP Coordinator
- Continued Stay of Residential Services - Residential Treatment Facility
The reconsideration request form must be completed and faxed to Acentra Health within the required timeframes, depending on the review type
