Provider Manual
The Provider Utilization Review and Quality Assurance Manual is a comprehensive guide for LTAC facilities. The manual contains information regarding policies, procedures and services that Acentra Health offers as Illinois' quality improvement organization.

Helpful Tips:
- CARE Tools must be uploaded within 13 calendar days after the patient is discharged.
- LTAC hospitals are required to submit tools for Medicaid and MANG Pend patients.
- To access eQSuite:
- From the homepage, scroll down to the bottom right side of screen.
- Click on the first eQSuite link located under eQHealth Web Systems.
